How the score works. Each theme carries a fixed weight (shown on it). Your theme score is the share of its checks met (Yes = full, Partly = half). Overall readiness is the weighted average across the themes you have answered — so unanswered themes never inflate or deflate it. Priorities are ranked by how much each unmet check is holding your readiness back. Structured around the national care-need domains and the four key characteristics (nature, complexity, intensity, unpredictability) that underpin CHC and enhanced-support decisions. It is a transparent measure of how complete your evidence is, not a prediction of the funding decision. Not a medical device.
